Apply for equipment
If you are a parent of a disabled child (or a therapist or personal assistant) you need to consider the following before applying:
- Is your child aged 18? (we need to receive your application before their 18th Birthday)
- Does your child have a permanent condition that affects their mobility?
- You have approached your local NHS wheelchair service and the mobility equipment your child needs is not available.

If you’d like to apply to Whizz-Kidz please download the introduction letter and application form using the links below and post the form to us once completed. To receive a hard copy of the application form or assistance in completing the form, please call our Children’s Services on 0845 052 1231 (Local rate).
If you are in a position to purchase the equipment privately, please go to other Equipment suppliers
