Step by step guide
Making an application
- To get an application pack call the Children's Services team on 0845 052 1231 and they will send you a hard copy of the form. If you would like to download and print the pack yourself you can do so at applying for equipment, where you will find the documents you need in PDF format for printing.
- Complete Part 1 of the form.
- Pass Part 2 of the form to your child’s local therapist to complete.
- Enclose a photograph of your child/children using their current equipment if applicable.
- Read and agree to the Terms and Conditions before signing the form.
- Make a copy of the application form and retain the Terms and Condition for your records.
- Once both sections are completed, please return it to Whizz-Kidz Head Office, the address is on page 2 of the application form.
We realise the application form is quite long, but we aim to get as much information from you at the start so that we can fully assess your child’s needs and later on avoid more form filling.
Important: We can’t proceed with your application unless every part of the form is completed.
What happens next?
- When we receive your application, our lead therapist will prioritise your child’s case
- Whizz-Kidz contacts you to confirm their priority level – fast track, high or medium.
Important: You need to keep us informed of any change in circumstances.
Funding your child’s equipment
- Whizz-Kidz will contact your local wheelchair/social/education service about joint funding.
- If local services can’t help other funding opportunities will be sought, including from other charities.
- We’ll also confirm whether you are able to make a financial contribution.
- Whizz-Kidz will regularly update you on your child’s case progress.
- When we have the funding available we will inform you that your child’s case is ‘open’.
Important: As Whizz-Kidz is reliant on voluntary funds we will only open a child’s case when the funding is in place. This can take time so we may need to wait. We appreciate this is not ideal, but will do everything we can to ensure your child’s case is open as soon as possible.
The assessment
- Once your child’s case is ‘open’ one of our local mobility therapists will contact you to set up an assessment appointment
- The assessment will include identifying:
- What other equipment is already in use.
- What is the best equipment to meet your child’s individual needs.
- When and how the equipment will be used at school, at home, for play etc.
- Aftercare requirements.
Getting the equipment
- Your child will be able to trial the equipment we recommend to make sure it’s right for them.
- The right equipment will be confirmed prior to placing the equipment order.
- If everything is in place the order will be made.
- We will recommend appropriate maintenance and insurance partners for you contact about getting cover for the equipment.
- When the equipment is ready, a mutually convenient delivery date will be agreed with you and your child.
- Our therapist and the local dealer will be at the delivery to ensure:
- The equipment is complete and set up correctly.
- You and your child are able to use the equipment.
- The handover certificate is completed.
- You have got appropriate insurance and understand the equipment’s maintenance needs.
- Your child’s wheelchair training needs are also identified.
- Any follow up will also be agreed.
